In 1218, documents attest to the passage of Saint Francis in Montalcino, and in 1285, Abbot Simone di Sant'Antimo donated the church of San Marco a Castelvecchio and the adjacent hospice as a perpetual inheritance to his monks. Adjacent to the church of San Marco, to the south, stood the church of San Michele Arcangelo, which was also donated a house, a vegetable garden, fields, and the entire Castelvecchio hill, a donation confirmed by Honorius IV in 1286. Both churches were demolished in 1287 to make way for a new, larger structure, 60 braccia long, 20 braccia wide, and 25 braccia high. Indulgences were granted to those who gave alms, but construction still progressed slowly. In 1334, the rector of the Ospedale della Misericordia bequeathed part of his estate to finance the construction of the churches of San Francesco and Sant'Agostino. The convent had received numerous donations: houses, shops, vineyards, and olive groves. Father Bonaventura Bovini left a detailed account and description of the convent's structure.
The interior of the church was completely renovated starting in 1788. It was Father Niccolò Fommei who, thanks to alms, undertook the modernization of the church according to a design by architect Tommaso Paccagnini.
The work, completed in 1794, completely changed the appearance of the church's interior, which became Baroque, depriving Montalcino of a precious artistic treasure, as Don Antonio Brandi wrote.
